Hospital Liaison-Weekends-Remote Qualifications & Responsibilities…
Foster collaborative relationships with hospital discharge planners and medical professionals.
Conduct assessments and evaluations of potential residents in hospital settings.
Coordinate smooth transitions for patients from hospitals to our nursing home.
Educate healthcare providers and community partners about our facility’s services and capabilities.
Act as a liaison between the nursing home and external healthcare entities.
Bachelor’s degree in healthcare administration, business, or a related field (preferred).
3-5 years Previous experience as an Admissions Director or Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital desired.
Strong communication and interpersonal skills.
Knowledge of healthcare regulations and standards.
